What is the registration process?

Before you complete the full dizziness questionnaire, you will first go through a short health screening and then register for Dizzy Care Network. The health screening is only a safety check. It is not the full questionnaire.

1. Complete the health screening
   You will answer a few safety questions about symptoms that may need urgent medical care, such as fainting, chest pain, trouble speaking, or sudden weakness.
2. Review any urgent medical warning
   If you answer “yes” to a safety question, you may see an urgent medical warning. Dizzy Care Network does not provide emergency services. If you think you may be having a medical emergency, call 911 or go to the nearest emergency room.
3. Register for Dizzy Care Network
   After the health screening, you can register. You will enter basic information such as your name, email address, birth year, zip code, travel distance, and sex assigned at birth. Some information, like your phone number and street address, is optional.
4. Check your email
   After you submit your registration, Dizzy Care Network will email you a link to complete the full dizziness questionnaire.
