Pre-Screening and Registration
These are the first steps in your journey with Dizzy Care Network.
You will complete a pre-screening to ensure that you are safe and do not need emergency medical care. If you receive the urgent medical warning, please contact 911 or go to the nearest emergency room or urgent care.
After you complete the pre-screening, you will get to register with Dizzy Care Network.
Questionnaire
This is a critical step in your journey to find care through Dizzy Care Network.
The questionnaire contains a variety of different questions that ask about your medical history, dizziness history, and more. This information is vital for matching you with the best possible care. Answer each question to the best of your ability.
Once you submit the questionnaire, clinicians will have 2 business days to review and match with you.
Select a Clinician
After the 2 business day period, or after all clinicians have responded, you will receive an email informing you that you can continue onto the next step in the matching process.
You will be provided a list of matched clinicians. Take your time looking at each clinician. When you are ready, select which one best fits your needs.
Once you select a clinician and submit your decision, you will see scheduling information. Please reach out to your clinician to schedule an appointment within the next 2 business days.
Expand Search to Find Match
If you do not match with a clinician, you will have the options to change your location or to expand your search radius.

In rare cases, there still may not be a clinician in our network who is the right fit for your symptoms or location. If that happens, we’ll let you know and continue to expand our network so more options become available. You can also check back later as new clinicians are added.
Direct Registration
This is not part of the matching process.
Direct registrations give each clinician a unique link they can share with care-seekers who already want to connect with them. When a you use that clinician-specific link, you can complete the Dizzy Care Network questionnaire before your visit. Then, a patient report that includes helpful symptom and history information is generated and sent to your clinician.
This can support better preparation, smoother scheduling, and a more informed first evaluation.
