When you join Dizzy Care Network, you will be asked to review and agree to the following Patient Consent Agreement. This explains the limits of our service, how we handle your data, and your responsibilities when using the platform.
IMPORTANT NOTICE FOR EMERGENCY SITUATIONS
The Dizzy Care Network is not an emergency service nor a substitute for urgent medical care. If you are experiencing serious symptoms, an acute health crisis, or a medical emergency, please contact your local emergency services immediately or seek urgent medical attention. Our platform is intended for non-emergency assistance and does not replace direct, immediate medical intervention.
LIMITATION OF LIABILITY
While we strive for excellence in assisting you, it is essential to understand that our service’s scope is limited to facilitating connections between patients and healthcare providers. The Dizzy Care Network does not practice medicine or any other licensed profession, and does not interfere with the practice of medicine or any other licensed profession. The Dizzy Care Network is not liable for any actions or omissions of independent providers, the accuracy of information they provide, any professional advice you obtain, or any outcomes resulting from their care. We recommend verifying any clinician’s credentials and considering all aspects before proceeding with any form of care.
DISCLAIMER OF WARRANTY
TO THE MAXIMUM EXTENT PERMITTED BY LAW, YOU AGREE AND ACKNOWLEDGE THAT THE SITES ARE PROVIDED “AS IS” WITHOUT ANY EXPRESS OR IMPLIED WARRANTIES OF ANY KIND, INCLUDING BUT NOT LIMITED TO MERCHANTABILITY, NON-INFRINGEMENT, SECURITY, FITNESS FOR A PARTICULAR PURPOSE OR ACCURACY. THE USE OF THE SITES IS AT YOUR OWN RISK. TO THE FULLEST EXTENT OF THE LAW, WE EXPRESSLY DISCLAIM ALL WARRANTIES OF ANY KIND, WHETHER EXPRESSED OR IMPLIED. None of Dizzy Care Network’s content should be considered medical advice or an endorsement, representation or warranty that any particular treatment is safe, appropriate, or effective for you.
GENERAL USE OF SITE
When you register on the Dizzy Care Network, you are required to create an account (“Account”) by entering your name, email address, and certain other information collected by Dizzy Care Network. To create an Account, you must be of legal age to form a binding contract. If you are not of legal age to form a binding contract, you may not register to use our services.
You agree that the Account information that you provide to us at all times, including during registration and in any information you upload to the Sites, will be true, accurate, current, and complete. You may not transfer or share your Account with anyone, or create more than one Account (with the exception of sub-accounts established for children of whom you are the parent or legal guardian).
The services are available for use by children (under 18 years of age) but children are not authorized and may not use the services without the supervision and verifiable consent of a parent or legal guardian. If you register as the parent or legal guardian on behalf of a minor, you will be fully responsible for complying with the terms of this agreement.
SHARING OF HEALTH INFORMATION
By engaging with our service, you consent to provide specific health information concerning your symptoms. This data is essential in connecting you with the most appropriate healthcare specialist. Your privacy is paramount, and we pledge to share this sensitive information solely with verified and licensed clinicians for your care facilitation.
INTEGRITY IN CLINICIAN RANKING
All associated service fees are uniform for all healthcare providers. Clinicians cannot pay to be featured or otherwise improve their rank in the matching process. Clinicians can only improve their rank by demonstrating their improved ability to care for dizzy patients. This includes activities such as completing additional training and certifications, getting additional professional experience in their specialty, and increasing their clinical capabilities. Any clinician trying to circumvent these measures will be removed from the site and barred from future participation in our services. Any qualified clinician who meets our participation requirements may join Dizzy Care Network; no eligible clinician is excluded.
TRANSPARENCY IN REFERRAL PROCESS
We operate on a transparent, mutual consent-based referral system. This means that while clinicians compensate us for the service of connecting them with potential patients, this occurs only after you, the patient, have consented to share your information with the specific clinician. Our role is to facilitate a connection between you and the clinician based on your unique healthcare needs and preferences, and the clinician’s expertise and capacity. Both parties must agree to initiate a professional relationship, ensuring a mutually beneficial match. There are no hidden affiliations influencing our matching process, and our primary commitment is to your informed choice and high-quality care. Clinicians who fail to maintain professional standards, attempt to misrepresent their qualifications, or otherwise violate our participation rules may be removed from the network.
USE OF ANONYMIZED DATA
We are committed to advancing knowledge and improving care for individuals with dizziness. By using our services, you agree that the health information you provide may be used in anonymized and de-identified form for research and quality improvement purposes.
All identifying details—such as your name, contact information, or any other information that could reasonably be used to recognize you—will be removed or coded in accordance with applicable privacy laws (including HIPAA, when applicable). Anonymized data may be combined with information from other participants and used in clinical research, publications, and presentations that contribute to the scientific understanding of dizziness and balance disorders.
To protect your privacy, we use industry-standard security measures, including encryption, secure data storage, and restricted access controls. Only authorized personnel with a legitimate need will have access to your identifiable information, and anonymization procedures are designed to prevent re-identification.
Your identifiable personal health information will never be shared, sold, or used for marketing without your explicit written consent. However, anonymized and de-identified information may be reviewed by Institutional Review Boards (IRBs), regulators, or other oversight bodies to ensure compliance with research and ethical standards.
We retain anonymized and de-identified data for as long as it is needed to support research, clinical advancement, and quality improvement. When data are no longer required, they will be securely destroyed or permanently de-identified in a way that prevents re-identification.
By continuing to use our services, you acknowledge and agree to this use of anonymized data.
DATA MANAGEMENT AND USER RIGHTS
The Dizzy Care Network is dedicated to maintaining the security and confidentiality of your personal data. We comply with all applicable data protection laws and will only retain your health information for as long as necessary to fulfill the purposes outlined in this agreement or as required by law.
You have the right to request access to, correction of, or deletion of your personal data. You also have the right to object to the processing of your data or request a restriction on processing, as well as the right to data portability.
For any inquiries or to exercise your data protection rights, please contact our support team. You may cancel your Account at any time by emailing us at support@dizzy.care. Upon cancellation, we may immediately suspend your Account. Subject to applicable law, Dizzy Care Network reserves the right to delete, maintain, or destroy all communications and materials posted or uploaded to the Sites.
Dizzy Care may suspend or terminate your use of the Sites, your Account and/or registration for any reason at any time. After such termination, we will have no further obligation to provide services.
Dizzy Care Network sites (“Sites”) are continually under development and changes to the Sites or services may be made at any time. We reserve the right to revise or remove any part of these terms in our sole discretion at any time and without prior notice to you by updating this posting. Any changes to these terms are effective upon posting to the Sites, unless otherwise specified. If you disagree with the terms, your sole remedy is to discontinue your use of the Sites or services. Your continued use after a change has been posted constitutes your acceptance of the changes.
GEOGRAPHICAL RESTRICTIONS
The Dizzy Care Network makes no representation that all products, services and/or material described on the Sites, or the services available through the Sites, are appropriate or available for use in locations outside the United States or in all states and territories within the United States.
By checking the box, you agree to abide by and consent to the terms of this agreement, and affirm all authorizations made herein. Further, you acknowledge your understanding and agreement with the Dizzy Care Network’s information utilization practices and the nature of our referral procedures.
